Esso Case Highlights Importance of Acting on Safety Concerns
Esso has been fined £1 million after a major gas leak at its Fawley refinery, with the HSE finding that known risks had not been properly managed.
The incident involved a large release of flammable gas after part of a pipework support structure failed. Nobody was injured, but the HSE found that corrosion had been identified years earlier and had not been properly addressed.
Known risks still need action
One of the key themes to emerge from the case is the difference between identifying a risk and actually dealing with it.
Many organisations have systems in place to record hazards and concerns, but those systems are only effective if actions are followed through. Risks that remain unresolved for long periods can become part of day-to-day operations, making them easier to overlook.
